We support participants whose conditions significantly impact daily living, independence and community participation. As a nurse-led provider, we place strong emphasis on governance, risk management, delegation oversight and staff capability to ensure safe, consistent and responsive care.

Inclusive Healthcare Australia delivers Nurse-led NDIS disability support and community nursing across Western Victoria.

We specialise in supporting participants living with:

• Acquired Brain Injury (ABI) and Stroke
• Progressive neurological conditions, including Multiple Sclerosis, Parkinson’s Disease and Motor Neuron Disease
• Early-onset dementia (NDIS-eligible)
• Participants requiring High Intensity Supports
• Individuals with complex, chronic or significant health needs

We also support participants transitioning from hospital or rehabilitation, those requiring complex daily personal activities, and individuals whose supports require experienced oversight and coordination.
