PACE Reporting Templates for Support Coordinators and Recovery Coaches
The NDIS has introduced new reporting templates for support coordinators and psychosocial recovery coaches so that the NDIS can:
- standardise provider reports and
- capture consistent information to help NDIA planners prepare for check-ins and plan reassessments with participants.

How do you know when reports are due?
You need to log-in to the my NDIS provider portal to track due dates for the reports.
How do I submit a report?
You can submit your reports by uploading them through your my NDIS provider portal.
If you are unable to upload via the portal, then you may send an email with the report attached to enquiries@ndis.gov.au.
Reporting Requirements
The templates ask for information about:
- The participant’s support needs and situation.
- The supports the participant is receiving.
- The participant’s progress in implementing their plan to pursue their goals.
When completing the report, Support Coordinators and Recovery coaches must:
- consult with both the Participant and their family/supports and
- make sure they are aware of the details that are included in the report.
How often are reports required?
Support coordinators and recovery coaches will be asked to submit reports across the life of a participant’s plan, generally at a set period.
The timing of progress reports may change, depending on the participant’s circumstances and when their next plan reassessment is due.
