Your Hart 2 Hart provide counselling supports in all areas of Melbourne and Gold Coast. Working in NDIS since government trial-site commenced in 2013  - we offer a unique service-delivery model whereby we support participants by considering their specific care needs first, then seeking services to meet needs, providing a model of true choice and control

Therapy Services

OCCUPATIONAL THERAPY - Initial Assessment, Functional Capacity Assessments, Cognitive /Adaptive Behaviour Assessment, ​GOLD COAST / TWEED

COUNSELLING SUPPORT:    Face-to-face or via phone (or skype / zoom).  Our counsellors are ACA accredited

PSYCHOLOGY  | Gold Coast   face-to-face / telehealth - at our Southport office.  Melbourne CBD: face-to-face / Telehealth (telephone and/or zoom/skype).
OTHER Australian-regions: Telehealth-only (telephone and/or zoom/skype)

Supports for: plan managed and self-managed participants | Payment conditions: Self-managed participants pay for supports within 1 week of providing the support | Plan-managed: invoiced after Support provided.
