Triniti Home Care is an NDIS organization founded in 2018 by a group of highly skilled Registered Nurses.

The organisation's directors, who are Registered Nurses with over 15 years of experience, work closely with clients to offer a holistic approach to healthcare.

We are dedicated to assisting individuals with disabilities in achieving independent living and receiving person-centered services. Our team of experienced professionals genuinely cares about each participant and takes the time to understand their unique situation, needs, and goals.

We create individualised Service Agreements and Support Plans to best support participants.

Services Provided

 

 

      

Assisting with Transport and Transport

  • Includes support that enable participants to build capacity to independently travel, including through personal transport-related aids and equipment or training to use public transport.
  • Work with clients to ensure that they can use transport safely by understanding the risks and ensuring the required mitigation measures are implemented.
  • Assess participants' ability to understand and manage the relevant risks and through our organisation's vehicle transportation services by ensuring the specific aids and equipment are placed to transport clients safely.

Community Nursing care for High Care Needs

  • Assessment of our client's past and present health conditions and the associated treatments used
  • Participant's support needs and the required frequency of this support
  • Hygiene measures,
  • Monitoring a participant's stoma (where present),
  • Enteral feeding requirements,
  • Tube feeding requirements,
  • Additional medical issues developed as a result of a participant's initial health conditions,
  • Risk factors that can lead to further complications,
  • Approaches applied to prevent or reduce health issues,
  • Ongoing health assessments,
  • Care and maintenance of medical and other equipment, and
  • Emergency medical support.

Development-Life Skills
Life Skills are the things we learn along the way that enable us to function daily in a complex world full of challenges, expectations, unseen obstacles, and unavoidable circumstances. People living with disabilities often have different experiences where they may not have had the same exposures, or they may face other barriers. In turn, they have a different set of life skills. Whether you or your loved one experiences a disability, comprehensive support services are available with funding from the National Disability Insurance Scheme (NDIS) to ensure you live your life to the fullest.
