Hello and Welcome to Open Hands Community Care!

Who We Are

Open Hands Community Care (OHCC) is a not-for-profit organisation started back in 2019 to contribute to the capacity building and transforming of lives through comprehensive and integrated Support Services. At OHCC, we're passionate about people, their choices and compassionate about their care. We're a Registered NDIS Provider supporting ALL participants (Plan, Self or NDIA Managed).

How We're Different

At OHCC, we understand that each participant's needs are different, and our Support Coordinators work with you to deliver support tailored specifially to your goals.

We specialise in working with participants from CALD Backgrounds.

  • Food Relief Service (by form of a discount groceries store, which is located on site at 318 Ritchie Road, Pallara Qld 4110 open from Tues to Sat from 10am.)

 

Support Workers

  • Needing help around the house, or want to learn a new skill? Whether wanting to get out into your community and make friends, or just be involved in social activities and life skills development programs, we can help!
  • We can provide you with the right Support Worker to assist you with daily tasks in your home and within the Community.
  • Our highly skilled, experienced and professional staff will support you emotionally, socially and provide practical ensuring your daily tasks and activities are met.
  • Our person-centred approach is what differentiates us from the rest. We tailor supports specifically to your goals, ensuring that each participant and their family are treated with Dignity and Compassion.

 

Accessing the Community

  • Whether you want to get out into your community and make friends, or be involved in social activities and life skills development programs, we can help by providing the right Support Staff that aid in this process.
  • We are committed to making a meaningful difference in participants lives by providing essential support and fostering a sense of belonging within community.
  • With dedicated Support Workers, we ensure individuals have opportunities to engage actively with surroundings and live fulfilling lives. We empower participants and clients to venture out into community, enriching their lives through meaningful experiences.

 

Support Coordination

  • We understand each participant’s needs are different. Whether you’re new to the NDIS or are in need of change, we are here to help you get the most out of your NDIS Plan.
  • Our Support Coordinators will help you understand your NDIS Plan better and assist you to connect with the right supports, health, accommodation and/or employment agencies as required, making the most of your NDIS Plan.
  • If you’re an individual with a Psychosocial Disability, our Recovery Coaches support you to take more control of life and better manage complex challenges of day-to-day living. Our team supports you to identify impacting gaps, assisting you navigate social barriers. Whatever the scenarios, we’ll assist you to achieve your desired goals.

 

Respite Care

We know that everyone’s situation is a little different, at Open Hands we tailor our Short-Term Accommodations and Respite options in our fully accessible homes designed to support family and individuals, so you can have a much-needed break. 

We provide 24/7 support from a team of experience, skilled and qualified Support Workers, supervised by experienced Health Professionals.

We have various accommodation services available - each tailored to provide a supportive and inclusive living environment where you can enjoy maximum independence and quality of life you deserve.
