Synergy Community Care

At Synergy Community Care, we understand that no two people are the same, and neither are their mental health needs. We specialize in delivering personalized, NDIS funded support for individuals with psychosocial disabilities across Northern Brisbane which includes most of the Moreton Bay region, and the Sunshine Coast. Our focus is on working closely with participants to craft tailored plans that help them build independence, engage with their community, and work towards recovery.
