Who we are

Hi. I'm Elorm. I am the director of Life With Choice Care (LWCC). Supported by my own deep experience as a support worker for many years in the welfare and disability sector, my team is dedicated to providing core and daily capacity building services to participants from all backgrounds and walks of life. 

My team is highly skilled, with strong experience in mental health, intellectual disability, and Autism especially, including support for teenagers with Autism.

In addition, we have experience with complex care and physical support using lifts, hoists and other assistive technology.

For participants / carers to take breaks from their usual routine and come back rejuvenated. Sometimes a change of scenery is necessary, and we are equipped to provide that respite, along with a homely environment to participants and at all stages. It gives them a chance to:

• Develop skills they’ve wanted to learn
• Make new friends, and try new things

Our support workers are trained professionals assisting participants of all ages to the best of their ability, whilst building their capacity. Services include:

• Complete assistance, personal care
• Food, Accommodation
• Daily activities (professional-approved) and more.

Accessibility/level of care needs supported: All levels of care needs supported, from low to high intensity care

Food and catering: All food and catering included. 

We will also provide planned social outings and activities for our guests to enjoy!

Accommodation Supported Independent Living (SIL) NDIS Melton

• In this accommodation our qualified and experienced support workers help and/or supervise daily tasks for participants to live independently while building capacities.
• Participants can use our Support Service - while living in a comfortable / secure home in shared settings
• Daily tasks include:
  • Cleaning, laundry, showering / dressing, toileting
  • Meal preparation, nutrition, new hobbies, medications, learning public transportation.
  • Dusting, brooming, mopping, dishwashing / drying utensils
  • Making beds, changing linen, laundry, folding clothes
  • Grocery shopping, other housekeeping
