About

     

Triniti Home Care is an NDIS organization founded in 2018 by a group of highly skilled Registered Nurses.

We provide a comprehensive range of services in the Victoria, ACT, and New South Wales regions. The organisation's directors, who are Registered Nurses with over 15 years of experience, work closely with clients to offer a holistic approach to healthcare.

In addition to physical care, Triniti Home Care is committed to enhancing their clients' self-worth and confidence.

We are dedicated to assisting individuals with disabilities in achieving independent living and receiving person-centered services. Our team of experienced professionals genuinely cares about each participant and takes the time to understand their unique situation, needs, and goals. We create individualised Service Agreements and Support Plans to best support participants.

 

Services Provided

Assisting Personal Activities

  • bladder and bowel management, and toileting;
  • attending appointments;
  • use of appliances and aids, communication and hearing devices;
  • being mobile at home i.e., support moving on and off the toilet and in and out of bed; or
  • basic first aid administered due to the participant's disability and application of splints.

Household Tasks

  • Meal preparation services:
  • allergies,
  • religious food requirements,
  • eating and drinking supervision requirements,
  • whether the participant can eat out
  • Whether the participant has a complex physical disability and requires positioning/turning of food to maintain airway safety and avoid choking and in pressure care

Services are undertaken in a way that maintains the safety and well-being of our clients, helping them to feel proud of their home environment.

      

Assisting with Transport and Transport

  • Includes support that enable participants to build capacity to independently travel, including through personal transport-related aids and equipment or training to use public transport.
  • Work with clients to ensure that they can use transport safely by understanding the risks and ensuring the required mitigation measures are implemented.
  • Assess participants' ability to understand and manage the relevant risks and through our organisation's vehicle transportation services by ensuring the specific aids and equipment are placed to transport clients safely.

Community Nursing care for High Care Needs

  • Assessment of our client's past and present health conditions and the associated treatments used
  • Participant's support needs and the required frequency of this support
  • Hygiene measures,
  • Monitoring a participant's stoma (where present),
  • Enteral feeding requirements,
  • Tube feeding requirements,
  • Additional medical issues developed as a result of a participant's initial health conditions,
  • Risk factors that can lead to further complications,
  • Approaches applied to prevent or reduce health issues,
  • Ongoing health assessments,
  • Care and maintenance of medical and other equipment, and
  • Emergency medical support.

Development-Life Skills
 
Life Skills are the things we learn along the way that enable us to function daily in a complex world full of challenges, expectations, unseen obstacles, and unavoidable circumstances. People living with disabilities often have different experiences where they may not have had the same exposures, or they may face other barriers. In turn, they have a different set of life skills. Whether you or your loved one experiences a disability, comprehensive support services are available with funding from the National Disability Insurance Scheme (NDIS) to ensure you live your life to the fullest.

Participate Community
What is Increased Social and Community Participation under NDIS?
 
This support category assists an NDIS participant in activities such as tuition, coaching, and skill building, such as art classes, sports coaching, and activities that build skills and independence.
 

 

Support Coordination
 

Support Coordination helps you navigate and achieve your NDIS plan and goals by connecting you to community support. Triniti Home Care enables you to maximise your NDIS plan by considering your specific needs and providing support around any challenges you may face. Our Support Coordination team understands your NDIS, gets the proper support in place, and assists in building your capacity for independence. We'll ensure you get service tailored to your needs and goal and work closely with you to unpack your plan and help you understand how to use it. We will guide you in selecting local supports that best meet your needs and achieve your goals and Negotiate service agreements between you and your providers. We will review your support services to make sure they are working well.
